Physicians determine which of their patients are appropriate candidates for available APHN programs and initiate the REFERRAL. Local participating facilities provide a warm welcome and comfortable implementation environment. Participants complete onboarding with a Program Trainer, and when all documents are completed, the final step is to place participants in a class schedule that fits comfortably into their daily/weekly routine. Classes commence and the Participant’s measured, monitored and motivated journey to better health, and for most a healthier life, begins.

Behind the scenes, the whole APHN ecosystem, including referrals, onboarding, documentation, records keeping, participating facility program operations, program trainer tracking, programs and programming oversight, and all program participants, is run completely and discreetly behind the scenes on an expertly crafted, highly customized, cloud-based Management System. On the front end, the Management System supports the commitment to better population health by guiding onboarding, maintaining required documentation, organizing programs and schedules, managing the connections and interactions between facilities, trainers and physicians, and much more.
The objective is not merely to get people into a facility. It is to create a fully functional pathway to reduced disease and improved health that begins with a clinical recommendation and continues through enrollment, participation, assessment, accountability, and demonstrable, measurable progress.
