APHN is designed to be outcomes driven. It is designed to measure what it does and learn from what it measures. Our goal is through measurement to determine what is working, what is not, what may be improved, and how the APHN models may be refined to achieve maximum adoption and optimum results.
The APHN Management System is structured to capture a variety of non-invasive participant measurements, to make those measurements visible to the participant, useful to the trainer, beneficial to the program, and when appropriately authorized, available to the participant’s physician.
Participants can see their metrics and monitor their progress through a unique log-in to the Management System presenting them with a simple visual presentation. The presentation will include medically accepted definitions of healthy ranges for each metric, and motivational messages designed to encourage, advise, or reinforce without presenting themselves as clinical diagnosis or treatment.
When participants have provided the necessary physician information and permission, summary Progress Reports can automatically be sent to the participant’s primary physician at the conclusion of each 12-week program. This creates a direct feedback loop between participants and the physician who initiated their program recommendation and referral.
APHN intends to build its evidence base over time through implementation and participant data. Program Data Reports may include participation, completion, and adherence statistics, program-specific metrics, healthcare utilization, cost impact, and other relevant factors.
The purpose of measurement is not to create a pretty success story. It is to determine what is actually working.
