The Components

Expand Reach and Impact

APHN is the operational framework that connects people, facilities, physicians, trainers and programs through a carefully designed infrastructure of processes, documentation, measurement, communication and accountability.

The proprietary APHN Management System is completely independent of any facility operating software and provides the operational environment through which APHN implementations are managed.

Physicians create the clinical pathway. Participating facilities provide accessible environments. Qualified fitness professionals deliver structured participant experiences. Training supports consistent implementation. Assessment and accountability create a feedback loop. Combined, they provide an environment for consistent participation leading to measurable and sustainable results.

Physicians

Physicians create the clinical pathway

Physicians are the cornerstone of APHN as they embody the First Line of Offense in creating a healthier America. Dr. Robert Sallis was Medical Director for Keiser Permanente when he introduced Exercise Is Medicine® more than 20 years ago, and ever since, Doctors have known that exercise and nutrition can play a significant factor in managing, reducing, even eliminating many common diseases. But they had no reliable way to implement proper exercise and appropriate nutrition for their patients.

Now they do. No longer will Doctors have to tell their Hypertensive, Diabetic or Obese patients “You need to lose weight, go to the gym” then hope for the best. APHN is the structure that provides reliable, accountable, clinically grounded and proven effective exercise programs specifically designed to address those three incredibly common yet highly manageable diseases.

Physicians refer their patients to local the appropriate APHN programs, and if there is not a program available locally, they can pro-actively work with Parks & Rec Rx Group to develop one… typically at a local fitness facility where the Doctor is likely already a member. If they desire, physicians may also collaborate with the Parks & Rec Rx Group Medical & Training Teams to develop the specific exercise protocols for their local facility.

Doctors can refer their patients to a local APHN program WITH CONFIDENCE because they helped design the medically directed exercise protocols and are probably also active members of the facility.

APHN is not trying to eliminate healthcare. We are trying to improve population health and reduce instances of many common diseases. Better population health = reduced dependence on healthcare.

Participating
Facilities

Participating Facilities provide the enviroment

For PARTICIPATING FACILITIES, APHN is designed to reach, engage and service a vast, untapped segment of their local population.

On the municipal side, APHN is an easily accessible programming model for population health. Designed to be purely Grant-Driven, Parks & Rec Rx Connect, Inc. is a fully I.R.S. recognized 501(c)(3) non-profit company with the sole mission of bringing APHN medically directed, disease specific programming to Parks & Rec District fitness centers, YMCA and Community Center fitness facilities, college & university gyms & health centers, and other municipal health & fitness entities.

Grant-funded APHN programs are structured to provide accessible, reliable and sustainable medically directed fitness programming to the facility’s local population – through physician referrals – at no cost to the facility or participants.

APHN’s commercial model represents a defined implementation blueprint for medically directed small group training. It is designed to create a valuable profit center and sustainable revenue stream driven by a unique business model that will be adapted to each facility’s existing staff, equipment and physical plant to ensure maximum utilization and optimum revenue.

Implementation financials are adjusted to be consistent with the host facility’s existing fee & payroll structure while ensuring that participating trainers are appropriately compensated, and the host facility will typically see a full return on initial investment in less than years. From then on, the APHN program should perpetuate and grow, the only cap to participation and revenue is the capacity of your building.

Qualified
Trainers

Qualified trainers facilitated structure and safety

All APHN programs are facilitated by nationally certified personal trainers, with active & current personal trainer certifications from one of a select group of carefully vetted organizations, and in addition, an advanced degree in exercise science or related field, specialized training credentials, and/or extensive relevant experience.

To ensure reliability, accountability and professionalism, all trainers must be employed by the host facility (not independent contractors), and are required to have current liability insurance, typically through the facility that employs them.

For the trainers, participating in APHN programming represents a valuable, viable and highly sustainable revenue stream. APHN Program Trainers have the opportunity to actively participate in the management and reduction of Hypertension, Type 2 Diabetes and Obesity on a grand scale, the lifestyle improvement of their program participants, and betterment of the health and wellbeing of the community in which they live and work.

Participants

Reduce disease instead of managing symptoms

DID YOU KNOW THAT MANY CHRONIC DISEASES, ESPECIALLY HYPERTENSION, TYPE 2 DIABETES AND OBESITY, CAN BE MANAGED, OFTEN REDUCED, EVEN OCCASIONALLY ELIMINATED WITH PROPER EXERCISE & NUTITION?

That’s right. Science has proven it, and your doctors probably know it, but the best they could tell you was “You need to lose weight. Go to the gym.” because there really weren’t any reliable options. Pre-packaged and “celebrity” programs are typically boilerplate and don’t address your specific needs, many facilities view your condition as an insurance risk, not a wellness opportunity, and studies have shown that trying to “go it alone” can be ineffective, unreliable, and potentially risky. Not anymore.

APHN medically directed programs provide your physician with a legitimate, reliable and sustainable solution based in proven science, not perpetual drugs. Specifically addressing Hypertension, Type 2 Diabetes and Obesity, your doctor can now refer you to these local programs with confidence, and in many cases your physician has participated in developing the exercise protocols you will be using.

Once your doctor has made the referral (don’t hesitate to ask for it), the host facility takes care of the rest. Make an appointment to begin onboarding, and the initial meeting will take about 30 minutes to complete some necessary paperwork. Your nationally certified and carefully selected program trainer will guide you through the forms, answer any questions, and begin your enrollment in your first 12-week APHN program.

If you are ready to be proactive in addressing and managing your disease instead of simply taking medicines for the rest of your life to manage your symptoms, then DO SOMETHING about it. Speak with your primary care physician and your local fitness facility about bringing APHN medically directed exercise programs to your community.

Better Health = Reduced Healthcare.

If you have been diagnosed with Hypertension, Type 2 Diabetes or Obesity, you can continue to let the pharmaceutical companies manage your symptoms, or you can take control of addressing your disease and your approach to wellness and in time, you may not need those drugs at all.

That is not a promise, but a goal. The promise is that if you do nothing, it changes nothing.

Programs

Medically Directed Programs For proactive wellness

The three foundational programs around which APHN was created specifically address Hypertension, Type 2 Diabetes and Obesity. All programs mandate a robust onboarding process to ensure both efficacy and safety, including program-specific onboarding documents. Hypertension and Diabetes programs meet twice a week, Obesity programs meet three times per week, and all are 12-weeks in length in small group training format with six participants per class.

All programs will be facilitated by nationally certified, carefully selected personal trainers employed by the host facility. Unlike most traditional small group training, a necessary component of all programs is the mandatory measurement of some non-invasive metrics. Each participant will have these measurements taken as scheduled, a few at the start and/or end of each session, some during each session, and several only at the start and end of each 12-week program.

These measurements are incredibly useful in helping participants and program trainers monitor individual progress, and each participant’s individual data may be reviewed by that participant through a secure online portal

The full scope of assessed data includes

  • Resting Heart Rate
  • Resting Blood Pressure
  • Resting Blood Ox
  • Exercise Blood Ox
  • Exercise Blood Pressure
  • Post Exercise Blood Pressure
  • Hemoglobin A1C
  • Body Fat
  • Weight

Not all metrics are recorded for all programs, however all measurements designated for a specific program will be recorded throughout the full length of the program.

Measurement

Metrics drive accountability and track progress

APHN programs represent the implementation of Dr. Robert Sallis’ Exercise Is Medicine® , a groundbreaking study that showed conclusively that many chronic diseases can be managed, reduced, even potentially eliminated through proper exercise and nutrition.

The APHN programs have been developed to specifically address Hypertension, Type 2 Diabetes and Obesity. To ensure safety, monitor efficacy, and motivate adherence, participation in all 12-week, medically directed programs includes the mandatory measurement of some non-invasive metrics. Measurements taken are determined by program design, measured under the watchful eye of the program trainers, and recorded in the secure APHN cloud-based Management System. Several metrics are taken at the start and/or end of each session, some measured during each session, and several taken only at the start and end of each 12-week program.

The resulting data can be incredibly useful in helping participants and program trainers monitor individual progress, and each participant’s individual data may be reviewed by that participant through a secure online portal. A measurement summary is also available for review to each participant’s primary care physician.
